Please have two physicians and one CRNA copmlete the Letter of Reference. Also, please sign both copies of your contracts. Keep one contract for your records and return one to us.

For your convenience, we offer direct deposit as a payment option. There will be no charge to you for this service. If you are interested in having your payments directly deposited to your bank account, please fill out the following PDF and return it to us by the Friday before the week you are next scheduled to work. If you wish to continue receiving your payments by check through the mail as usual, please do nothing.

*If you choose to take advantage of the direct deposit option please read the following information carefully:

• We MUST receive your billing invoice by the WEDNESDAY FOLLOWING the week you worked.
• If we receive your billing invoice by the Wednesday following the week you worked, your money should be available in your account the morning of your pay date.
• You will still receive a bill payment stub in the mail to verify the amount deposited into your account.
